House Call Service

House Call Practices have been around for centuries. House calls became more available here in the DFW metroplex as part of the reform bill passed in 2010. It was to assist with patients who have chronic ill, difficulties with activities of daily living and a history of frequent hospitalizations and emergency room visits.

Call, email or fax patient demographic and clinical information available
Clinical information evaluated for appropriateness
Patient/family called
Appointment scheduled with in 3-7 days
Communicate with the referral source with update on referral
Complete assessment
Complete initial diagnostic testing when appropriate
Send all documentation which is appropriate to the referral source
